If anyone else is interested in responding to the forms that I posted,
please do not send me any emails. I am going to be away from home/my computer access for an unknown amount of time and I will not be able to compile this information.
I hope that someone else will want to use this format for its designed purpose of complitialion.
Two board members responded by sending me emails, please resend to whoever decides to take on this assignment.
